VISIT THE OFFICE
Come into the WCCAE office (address below). The fee is payable in person with cash, check, or money order. (Sorry, no credit cards.)

MAIL IN YOUR REQUEST
Mail in your request with the information below, plus check or money order, and signature. No cash in the mail please! See address below.

REQUIRED INFORMATION
• Your last name, first name
• Birthdate and last 4 digits of your Social Security number
• Name under which you registered for classes, if different from above
• Your street address, city/state/zip code
• Your current telephone number, in case we have a question
• Any other names usd while enrolled
• Address where transcript is to be sent:
xxxName of school, agency or company
xxxAttn: (person's name)
xxxStreet address
xxxCity/State/Postal code/Country if not U.S.
• Dates of attendance, from when to when
• Program taken: GED, day classes, evening classes, medical, computers, etc.
• Year graduated
